Complete Medical Forms.
*New Scouts must complete Step 1 right away. A "new" Scout is anyone who has not been registered with Scouting America within the past 12 months. Returning Scouts will need to to renew and pay the yearly National Registration fee prior to the Scout’s current expiration date. (Please login to your myscouting.org account to check your current expiration.)
Create an account (or login) with Scouting America at the link below, and complete registration for your Scout:
Scouting America Registration (Boy Scout Troop 315, St. Anastasia CYO).
Scouts returning from the previous year do NOT need to complete this step.
Families returning to Scouting with a new Scout still need to complete Step 1 but must use their existing BSA account information to register.
Send us an email using this link (Boy Scout Troop 315) to let us know you've registered (so we can look out for your application). Also please let us know any email addresses you would like to have added to our troop email distribution list.
Complete and print the medical form found at the link below. This form is used for "All Scouting Events" and activities less than 72 hours. Additional forms will be required if / when your Scout participates in events lasting more than 72 hours (for example, a week at summer camp). These medical forms are held in confidence by troop leadership, but taken to all overnight troop events in the event that medical attention for your Scout is required.

Bring a printed copy with you to your first in-person meeting. Scouting America prohibits digital submission/ retention of these forms.
Please remember to accurately fill in all information AND attach a copy of your insurance card.
NOTE: We highly recommend you fill out this form electronically and then save a copy to your personal computer, as much of the information can be reused from year to year.
