Application Packet

Nominees: All application forms are to remain confidential, and to be shared among the members of the Board only.  Information relating to a nominee's career statistics, records, achievements, etc must be supported by accompanying documentation of sources cited so that the information may be verified independently.  All application forms must be received by the athletic department (online or the paperwork) by September 14th of the current year, in order to be considered for induction into that years class.
