Once you have met the prerequisites listed on the home page, please contact me FIRST, and then fill out the other intake forms. These forms are designed to help me know where you are, where you want to be, and to cover my @ss. I know it seems like a lot of filling out, but it's worth doing! I am happy to help you with the forms if doing the forms is an obstacle for you! I know what it is like to feel put off by doing paperwork!
