To participate in Welcome Week events, you’ll need to complete a waiver or have a parent/guardian complete an informed consent form for you.
18 years old or older: You can sign the waiver yourself.
17 years old or younger: A parent or guardian must sign the informed consent form for you.
Once your form is completed, keep an eye on your McMaster email for your personalized QR code — this will be your ticket into all Welcome Week events!
We’ll be checking your QR code at our event check-in tables, so please have your form completed before Monday, August 31st, the start of Welcome Week.
