THINGS TO REMEMBER REGARDING THE BINANGONAN CARE CARD

REGISTER ONLINE!!!

Simply follow the necessary steps:

1. Visit our Official Website at...

www.binangonancarecard.com/register

2. Fill-out the necessary information and make sure that all the data are valid and true.

3. If everything is already complete, click the Submit Button.

4. Make sure that you have copied the reference number that the system will give you.

5. Go to the Binangonan Care Card Booth at the Municipal Hall of Binangonan, bringing with you two (2) copies/photocopy of your Valid IDs.

6. Pay the amount of ₱220.00 and make sure that the staff will take the following: Photo Capture, Digital Signature, and Biometric.

7. Wait for a phone call or text message from us in order to claim your Binangonan Care Card.

List of Valid IDs accepted in availing Binangonan Care Card.

1. Bring one (1) of the following Primary Valid IDs:

*Philippine Passport ID, *Driver's License, *SSS UMID ID, *GSIS eCard, *PRC ID, *IBP ID, *OWWA ID, *Diplomat ID, *OFW ID, *Senior Citizen ID, *Voter's ID, *GOCC & Gov't Office ID

or two of the following Secondary Valid IDs:

*NBI Clearance, *Police Clearance, *Barangay Clearance, *Cedula (CTC), *Voter's Identification (VR), *Gov't Service Record, *School ID (Present), *School Form 137, *Land Title, *PSA marriage certificate, *PSA Birth Certificate, *Seaman's Book, *Old Postal ID, *Philhealth ID, *PWD ID, *TIN Card, *Firearm's License Card, PLRA Company ID, Alumni ID

Note: If your Primary ID does not have your written address, you may bring one of the Secondary Valid IDs with an address as mentioned above.

FREQUENTLY ASKED QUESTIONS (FAQs)

WHAT IS THE PURPOSE OF THE BINANGONAN CARE CARD?

1. This is proof that you are a resident of Binangonan.

2. Included in the services are Accidental Death / Accidental Dismemberment Insurance worth ₱10,000 from BenLife.

3. You will also have discounts from Generika Pharmacy when you buy your generic medicines. You will get a discount of 5% for purchasing P300.00 worth of medicine, and 10% for ₱500 and above.

4. If you will have laboratory tests in PAG-ASA Hospital you can avail of a discount from 20% to 30%. This includes Normal and Cesarean Packages.

5. If you spend worth ₱250 and above from Jollibee Binangonan, you will get a free Peach Mango Pie or Regular Fries for every transaction.

6. The Card that you will receive can also be used as an ATM Card from Binangonan Rural Bank.

IS THERE AN EXPIRATION DATE FOR THE BINANGONAN CARE CARD?

1. Every year, the insurance has to be renewed for ₱50.

2. Every 3 years, the Card has to be renewed.

3. If our Care Card still has money, everything will be transferred to the new Card that you will avail.

HOW MUCH IS THE RENEWAL OF THE CARD?

1. ₱220 subject to change in case there is an adjustment.

WHAT IS THE PURPOSE OF ERX, DENGUE COVERAGE, ACCIDENTAL REIMBURSEMENT, AND FAMILY HEALTH CARD?

1. ERX - Emergency Room Expense, this is being paid for ₱330 every year. For the Binangonan residents who availed of this benefit and happened to be sent to the Emergency Room, he/she can have a reimbursement of up to ₱5,000.

2. Dengue Coverage – this is being paid for ₱330 every year. For the Binangonan residents who availed of this benefit and happened to have expenditures related to Dengue case, he/she can have a reimbursement of up to ₱5,000.

3. Accidental Reimbursement – this is being paid for ₱330 every year. For the Binangonan residents who availed of this benefit and happened to have an accident-related case, he/she can make reimbursement of up to ₱5,000.

4. Family Health Card – this is worth ₱1,499, the card holder and 2 other members of the family can avail unlimited consultation in all FAMILY DOC CLINICS nationwide.
