Diagnostic/Evaluation Services

I.E.P. Facilitation
● Compliance - Activities to ensure compliance may include but not limited to:
o School improvement file review if requested.
o Model samples for specific disabilities.
o Provide updated information on IEP compliance issues.
o Attend IEP meetings if needed.
o Consultation with process coordinator as needed.
● Resource person(s):
o Technical Assistance-Coop staff will assist districts in issues and situations relating to Special
Education children, staff, programs and families.
o Section 504-Coop staff will provide assistance with schools dealing with 504 students and in
the development of those plans.
o Co-op staff will serve as school advocates as needed.
Professional Development for Special Education Staff
● Annual training for district special education teachers, therapists and coordinators regarding compliance issues, best practices, IEP development and current issues in special education.
● Annual training for district administrators regarding compliance issues, discipline, legal requirements and current issues in special education.
School Psychological Services
● The Co-op school psychologists provide technical assistance and support to assure to appropriate referral of students for evaluation and eligibility determination.
● The school psychologists administer & interpret appropriate assessment instruments, conduct observations, and data collection as determine by the review of existing data team review.
● The school psychologists assist the districts in the planning and providing behavioral supports to students who present behavioral or disciplinary concerns.
School Social Work Services
● The school social worker administers & interprets appropriate social emotional and behavioral assessment instruments, conducts observations, and data collection as determined by the review of existing data team review.
● The school social worker assists the districts in the planning and providing behavioral supports to students who present behavioral or disciplinary concerns.
● The school social worker will provide support and technical assistance to member district families on the acquisition of educational, social, medical, mental health or economic assistance services to ensure that a full range of family support services are made available to children with disabilities.
Annual Transition Fair
● The Co-op will organize and facilitate a “Cooperative Transition Fair” on an annual basis.
1. Contact and secure vendors to attend the annual event.
2. Contact and secure location of event.
3. Contact local business for possible product donation.
4. Collaborate with cooperative districts on student attendance.
Jasper Intensive Classroom
● The Cooperative provides an intensive classroom for students with behavioral challenges in Jasper. This provision includes a self-contained classroom teacher, classroom supplies, case management, teacher personnel development and mileage expenses. Each district is responsible for providing personal paraprofessionals for the provision of services to an individual child. Sending districts will provide OT, PT and SLP services to students as needed. If inclusion in regular classes is required on a child’s IEP, the sending district will provide these services. The sending district will provide transportation to and from the classroom.
Early Childhood Special Education (ECSE)
The cooperative is responsible for the evaluation, identification, placement, direct services and case management for all students ages 3-5 who are eligible for special education services. Each district will serve as the LEA in all decisions, however, the cooperative will provide all services needed for ECSE students.
Placement options are as follows:
o Early childhood setting (preschool/Head Start)
o Early childhood special education Classroom
o Home
o Part-time early childhood/Part-time early childhood special education
o Residential facility
o Separate school
o Itinerant service outside the home
● All ECSE student data is to be reported by the Cooperative Fiscal Agent and all ECSE expenditures will be applied for reimbursement and received by the Cooperative Fiscal Agent with the exception of transportation cost. Sending districts providing transportation cost will apply for reimbursement and receive those funds directly.
ECSE Classrooms
● The cooperative provides one ECSE classroom in Lockwood. Classes are half day sessions (8 a.m. - 11 a.m. & noon - 3:00 p.m.), Tuesdays through Fridays.
● This provision includes an ECSE classroom teacher, speech/language services, classroom supplies, case management, teacher personnel development and mileage expenses. Paraprofessionals and additional therapies outlined on a student’s IEP will also be provided by the Cooperative. The sending district will provide transportation to and from the classroom.
RTI Consultation and Monitoring
● The cooperative provides technical assistance and consultation to districts utilizing the RTI process. Assistance may include: direct intervention services, progress monitoring and data collection, forms development, goal development.

● The Cooperative maintains a website that provides sample forms and resources for special education staff and administrators. Forms include:
o All special education required forms
o Discipline Forms
o Sample 504 forms
o Sample RTI forms
DESE requirements
● The Cooperative will assist districts with the following requirements:
o Part B Application
o Part B Final Expenditure Report
o Monthly Payment Requests
o ECSE Final Expenditure Report
o High Needs Fund Application
Public Notice
● The Cooperative will arrange for the Public Notice to be placed on various local radio stations. The Coop will provide documentation when the ad has been run.
