We accept insurance and selfpay via cash or credit card.

CAQH insurances such as Aetna, BCBS, Cigna, Humana, United Healthcare, Web-TPA, etc. are accepted here (PPO insurance).

Please call your insurance before arriving for your appointment to confirm your benefits, payments, deductible, etc. Some insurances will not cover your visit and/or additional services.

Medicaid and Medicare are accepted but we are not accepting new patients with Medicaid and Medicare.

Medicaid is only accepted for people 18 years old and younger.

We are not currently accepting new patients with Superior Health Plan.

We take cash and credit card in office.

New patients first appointments are $120 upfront, not including possible additional services that may be deemed necessary by the provider. Established patient appointments are $100.

Payment can be processed for appointments (walk-in or scheduled) by calling from your car in the parking lot.
