Please complete the Alief Veterinary Science Animal Prescreening form. Students will review your submission and contact you within two school days to confirm availability and provide next steps.

To maintain a safe and sanitary training environment, the following requirements must be met prior to service:
• Pets must be in good general health
• Dogs and cats must be current on required vaccinations
• Pets must be on current flea prevention
• Proof of vaccinations and flea prevention is required prior to drop-off
• Pets must arrive secured on a leash or in a carrier
Drop-off and pick-up instructions will be provided when your appointment request is confirmed.

All required medical records must be submitted prior to service. If records were not submitted in advance, please bring a printed copy or have a digital copy available at check-in.
