Purpose
The purpose of the BSID-4 is to evaluate the developmental functioning of infants and toddlers ages 1 to 42 months. It is used to identify developmental delays, support early diagnosis, guide intervention planning, and monitor progress across key domains such as cognitive, language, motor, social-emotional, and adaptive behavior.

Children aged 16 days to 42 months (approximately 0 to ~3.5 years)
Early intervention/home-based services
Pediatric clinics and outpatient therapy
Preschool or developmental follow-up clinics
To identify developmental delays, assist in planning interventions, and monitor developmental progress in infants and toddlers.
Assesses five domains:
30–70 minutes, depending on domains administered
Administered individually; caregiver present but does not assist
Scales can be administered separately or together
Standardized instructions; available in print or via Pearson’s Q‑Global digital platform
Qualified clinicians (e.g., psychologists, SLPs, OTs, developmental pediatricians) after completing Pearson-provided training
Examiner manual
Stimulus books and manipulatives
Record forms or digital access via Q‑Global
Online or paper scoring/reporting tools
Responses scored 0/1/2, converted to raw, scaled, and composite scores; yields developmental quotients, percentiles, and age equivalents. Digital scoring available via Q‑Global
Normed on national sample (updated for population changes)
High reliability and validity; BSID‑4 shows improved sensitivity compared to earlier editions
Strengths
Comprehensive coverage of five developmental domains
Can administer individual domains flexibly
Up-to-date norms & strong psychometrics
Supports early identification & intervention planning
Efficient digital administration and scoring
Weaknesses
Costly initial investment
Requires clinical training
Administration time may be lengthy for full battery
Caregiver proxy reports may introduce subjective bias
