Dear Participant,
You have been asked to participate in a study being done for my ENG 102 Honors class at Cleveland State University. The purpose of this study is to obtain information regarding the relationship between increased social media usage and body dissatisfaction. We want to study whether or not social media usage increased during the Covid-19 quarantine, as well as whether or not that hypothetical increase also caused an increase in body dissatisfaction in young adults aged 18 to 22. You have been asked to complete this survey because you are a social media user between the ages of 18 and 22.
You will be asked to sign this consent form in order to participate in an online survey. This should only take about 15-20 minutes of your time. The online survey asks a series of questions regarding yourself, your social media usage, and the way you feel about yourself. Participation is voluntary. You may withdraw at any time without penalty. There is no reward for participating. Your answers will be confidential and only I will have access to your answers. Your name will not be used.

Considering this version of my informed consent form was the version that went out to all the participants in my survey, I felt it unnecessary to revise that. However, the table below it- which includes a comprehensive list of all my research questions- did need some revisions. The language in it was originally very informal, so I edited that a little bit to seem slightly more professional. This table was also originally organized through a Google Document, but in order to fit on this website as a table, it needed to be put into a spreadsheet.
This research activity helped me to write the informed consent form that I would eventually distribute to the participants in my study, and it also forced me to think deeper about the questions I was asking and what kind of data they would give me. In addition to that, it helped me organize my thoughts into one table so that I could figure out how each question on my survey was going to help me answer my research question.
