New Student Entrance Requirements

All new students must have a completed Health Survey, Dental Form, Physical Examination & a list of all immunizations with dates of administration as described below. If you have any questions please call the School Nurse, Patricia Black at (315) 416-1956.

Part I - Physical Examinations

The New York State Department of Health requires that for new each student, a copy of a current physical examination is provided. A current physical exam is defined as an exam that has been dated and signed by your child’s Health Care Provider and completed no earlier than within one year prior to the beginning of the current school year’s starting date. Exam’s completed prior to that date will not fulfill the requirements.

It is suggested that the Physical Exam be done by the family physician who best knows your child. The family physician is better able to judge any changes or deviation in the child’s state of health. Findings can be discussed and referrals (i.e., eye glasses) can then all be made in one visit. Physical Examination Form.

Part II - Immunizations

Immunizations are an important part of our children’s health care because they protect against vaccine preventable diseases. The Public Health Law of the State of New York requires that all children entering school must be fully immunized against the most common preventable childhood diseases. As such, students must provide a record of all of the required immunizations by the first day of school from the child’s healthcare provider. New York State requires that any student lacking immunizations must be EXCLUDED until the school receives proof of vaccines or medical or religious exemption.

Required Immunizations:

  • 3 doses DPT (Diphtheria, Pertussis, Tetanus)
  • 3 doses TOPV or IPV (Polio)
  • 2 doses MMR *(Measles, Mumps & Rubella) administered no earlier than the date of first birthday.
  • 3 doses Hepatitis B
  • 1-Varicella (Chickenpox) administered no earlier than the date of first birthday or Physician diagnosed and documented proof of disease history
  • For students entering 6th Grade, a Tetanus, Diphtheria and Pertussis Booster (Tdap) is mandated by NYS law.

If you do not have insurance, the Onondaga County Immunization Clinic, located at 301 Slocum Ave., Syracuse, NY 13204, is open every Thursday, 8:30 a.m.‑ 11:30 a.m. They can provide your child with his/her shots.

You may either send the completed physical exam and a copy of the child’s immunization record to the school or have you Health Care Provider fax a copy. It is a good idea also, to save a copy for your records.
