Terms and Policies (Your Rights and Responsibilities)
You have certain rights and responsibilities as a patron at ASCENT. These rights and responsibilities will help you take an active role and help you promote your health and well-being. Please read the following information and ask your provider if you have any questions.
You have the right to
be treated with respect, consideration, and dignity without discrimination based on race, color, creed, religion, national origin, gender, age, marital status, disability public assistance status, veteran status, sexual orientation, gender identity, or gender expression.
know the names and credentials of the practitioners serving you.
be referred to by your preferred name and pronouns.
confidentiality and privacy with respect to your preferences (cultural/social) and for all communication/documentation via phone calls and emails from practitioners at ASCENT (Exception: as allowed and required by law, your written permission is required before we may give information to anyone)
receive services voluntarily in a safe setting, free of abuse or harassing behavior.
know the beneficial effects and potential unfavorable activations within receiving services.
be informed of services available at ASCENT and any related costs.
know what provisions are available for after hours and emergency costs.
consent to, or refuse, any care or treatment.
select and/or change your service practitioner.
review your records with your service provider..
receive appropriate referrals to other service providers.
receive information in a manner you understand. (language interpretation, translation, visual, and hearing accommodation services are not provided and are the patron's responsibility).
You have the Responsibility to
respect ASCENT policies.
be respectful to all staff, as well as other patrons.
be an active participant during your services.
provide complete and accurate information to the best of your ability about your health and wellbeing if relating to your services.
give correct personal and financial information and pay prior to services as ASCENT financial policy requires.
ask about anything you do not understand.
report any significant changes in symptoms or failure to improve.
keep appointments or cancel within 24 hours of your scheduled time.
call 911 or 988 if you are experiencing an emergent emotional health need.
If you wish to have a detailed copy of the Health Care Bill of Rights, please ask one of your practitioners to provide you with one (MN Stat. 144.651).
