Program Overview

Delivering great care to our patients has always been at the core of AltaMed’s mission and, as part of this commitment to “Quality Care Without Exception,” we are pleased to offer support and resources of our Quality Improvement Department to the practices within AltaMed’s contracted provider network.

Quality performance measures (i.e. HEDIS, CMS 5 Star) are important indicators of our patient population’s health care access and outcomes. AltaMed uses data to track our performance to ensure we deliver high quality care to our patients and fulfill our mission of eliminating disparities in health care outcomes.
