PTA Scholarship Application

Previous Year's Instructions:

Instructions - Applications are due by March 20, 2023 at Noon.

1. Student applicants must be a PTA member, enrolled in a high school PTA/PTSA in good standing.

2. Download Application Form and Reference Form.

3. Open the Application Form and Reference Form using Acrobat Reader to electronically fill in the information.

4. Print out Application Form and sign it.

5. Scan the documents and use the Upload Documents link below to submit all of your documents.

(High School Counselors can help with printing and scanning the document.)

Upload Documents - upload all related documents (Application Form, Reference Form, Letter of Recommendation, Personal Statement Essay, Transcript, Proof of PTA Membership) to complete submission
