Student applicants must be a PTA member, enrolled in a high school PTA/PTSA in good standing.
Download Application Form and Reference Form.
Open the Application Form and Reference Form using Acrobat Reader to electronically fill in the information.
Print out Application Form and sign it.
Scan the documents and use the Upload Documents link below to submit all of your documents.
