Here's the Process:

1. Complete this application.

2. Submit documentation of diagnosed disability. You may do so via the link at the end of this application. Please see our website for more information regarding documentation requirements. If you do not have documentation or do not know how/where to obtain it, please contact us prior to completing the application.

3. After your application is reviewed, you will be contacted by a staff member for the next steps. If you do not hear from someone within 7 business days, please follow up to inquire. 361-825-5816
