Performance Measures

When you go to the tab for Performance Measures, you will see a snapshot of the performance measures associated with your grant as well as each measure's target value.

To see more information about each measure, click on the link under Grant Metric Name. This page will give you more information on each measure including the focus area, interventions, and performance measure type. You will also be able to see Performance Measure data submitted through your Mid-Year and End-of-Year APRs once they have be submitted and made available to view by your Program Officer.

To navigate back to the Grant Details page, click Grant at the top of the page.

Program Amendments

On the amendments tab, there will be a snapshot of all amendments associated with your grant and their status.

To see more information about each amendment, click on the link under Grant Amendment Name. This page will give you more information on each amendment including the status, description, and any files associated with the amendment.

Please note: You should obtain approval from your Program Officer prior to submitting a grant amendment.

To submit a new grant amendment, please use the following steps:

  1. At the upper right of your Grant page, click the button that says New Amendment in the upper right. Clicking this button will open a new tab where you can fill out a grant amendment form. Your grant name should automatically populate into the form.

  2. Select the type of grant amendment you would like to submit. The types of grant amendments are:

      • Budget Change: Use this amendment to make changes to your grant budget. Selecting this option will redirect to Budget the section. Please note this section will only be visible if you have budget permission.

      • MSY Expansion: This amendment allows you to request an expansion of your Member Service Years.

      • MSY Reduction: This amendment allows you to request a reduction of your Member Service Years.

      • Member Slot Conversion: Use this amendment to request a reconfiguration of your member slot types.

      • Duration of Service Waiver: Allows you to request a duration of service waiver for member(s).

      • Disaster Requirement Waiver: Use this amendment to submit a disaster requirement amendment.

      • Service Site Change: Allows you to make a change to your service site(s).

      • Performance Measure Change: Use this amendment to make changes to performance measures.

      • No Cost Extension: Allows you to apply for a No Cost Extension.

Please note that some amendment types will also require a budget change to be submitted. The amendment form will include a reminder for amendments that also require a budget change. These amendments will not be considered until a corresponding budget change is submitted. Please see the Fiscal page of this website for more information on submitting a Budget Change.

  1. Complete the form according to the directions and submit.

      • Once submitted, you will receive a copy of your responses in your e-mail.

      • This is who will be notified: chart of which Grant Contacts notified

  2. After an amendment is submitted, you can click the Amendments tab to view its status. The amendment will be named by the type of request and the date submitted. Statuses include:

      • Submitted

      • Approved

      • Rejected

      • Withdrawn

  3. Your Program Officer will reach out to discuss any clarifications and/or additional actions needed for the amendment.

  4. Once the amendment is resolved and approved, the finalized request and clarifications will be attached as a PDF file on the Amendment record.
