The purpose of the Utah State Developmental Center is to provide an effective, efficient array of critical services and supports that promote independence and quality of life for Utah's most vulnerable people with disabilities in partnership with families, guardians, and the community.
The place is very big with multiple buildings. All the buildings have residents, caregivers and nurses. Total employee count is estimated at around 620 people.

We will be working with the human resources department to investigate the following issue:
What can we do to increase the retention rate of potential and current employees?
We need to understand where the company stands at this moment. To do this we will start by focusing on the following:
Review current hiring process
Review current employee job satisfaction
Review data of turnover rate
Business structure:
What is the hierarchy of the business at this time?
The previous question is crucial. It may determine a question in our survey so we can find if there are differing opinions of job satisfaction within the different departments. We will be creating a flowchart of what level reports to what level(s) in the company.
How many total employees does the company have at this time?
Hiring process:
What kind of structure is in place to find, interview and hire qualified individuals at this time?
We want a break down of what the interviews are like (from managements perspective). Do they ask the same questions in every interview? Do they have to immediately write a report on the interview? How many interviews have to be made prior to approval to hire?
Job satisfaction:
What is used at this time for measuring employee job satisfaction?
On that scale (if no scale is used yet, use 1-10) where does the company want job satisfaction at?
Who evaluates the information gathered? Is this information shared with key stakeholders/decision makers?
Is there data that can be made available to the 5a Team, showing what the turnover rate is of the company at this current time?

What is the general feeling that the company seeks to promote in their places of business?

Who evaluates the information gathered? Is this information shared with key stakeholders/decision makers?

The purpose of this is to get the perspective of employees who are currently leaving the company to get candid information from individuals.
