To move forward in the next phase of the application process, please complete the following pre-employment documents at your earliest convenience. Once emailed back to us, we will proceed to the next phase of your application.

Instructions

Download the blank pre-employment documents provided.
Fill in all highlighted fields, sign, and date each document.
Please ensure that you use your legal name exactly as it appears on your government-issued ID for all pre-employment documents .
Save your document using the naming convention listed by each blank document.
Once you have completed and signed all documents, reply to the same email invitation you received for completing the pre-employment documents and attach all completed files.

Important

All documents are fillable PDFs. We recommend downloading each form, completing it, and submitting it in PDF format to help expedite the review process.
- If you are using a Windows computer, you may use the built-in Microsoft Edge browser to complete the documents.
- If you are using a Mac, you may use the built-in Preview application to complete the documents.
- Alternatively, you may use DocHub to complete the documents for free.
If you prefer to print the document and complete it by hand, JPG, JPEG, and PNG file types are also acceptable for submission. Please ensure that your handwriting is clear and legible.

5. At-Will Status Form

This document acknowledges the Student Intern job classification as an At-Will position.

6. W-4 (Federal Employee's Withholding Certificate)

This document computes the amount of federal income tax to deduct and withhold from the student intern's pay.

Important: For Steps 2–4, consult your parents or a tax advisor if they apply to your situation.

7. DE-4 (State Withholding Allowance Certificate)

This document computes the amount of taxes to be withheld from your wages to accurately reflect your state tax withholding obligation.

8. ID/Transportation Pass Agreement

All TCAP student interns will be issued a badge containing unlimited TAP fare. This document lists the conditions of its usage.

9. Beneficiary Designation Form

This designation allows you to transfer assets directly to individuals, regardless of the terms of your will. Beneficiary designations are often made when a financial account, retirement account, or life insurance policy is established.

This memo outlines the policy for disclosing employee information under the California Public Records Act (CPRA) at Metro.
