Application Package Checklist

Your application package (zipped into a folder and labeled correctly) should contain the following:

1. Application questionnaire

2. Results from the 4 personality tests:

   • • 16 Personalities Test

     • The VIA Character Strengths Survey

     • DISC Personality Test

     • Beblin Test

3. 2024/25 portfolio analysis document

4. Manifesto with the budget included

5. Any key tasks in the portfolio-specific section

Also include the following letters:

1. Signed release letter

2. Signed parental consent letter

3. Signed endorsement letter from your current LCVP

4. A copy of your most recent academic transcripts
