Applicants must submit a 500 word essay. Essay must be the original work of the applicant and typed. Topic choices include, but are not limited to: Education, Health and Wellness, Economic Empowerment, Mentoring, Peer Pressure, and/or Movement of Youth (MOY) program.
Photo must be a high-resolution jpeg file suitable for publication (i.e. cap and gown head-shot, or other professional quality and professionally dressed photo).
Transcript must include all pages, show GPA on a 4.0 scale, and be legible.
Applicants must submit acceptance letters from the institution he plans to attend.
Applicants are required to have performed fifty (50) hours of community service within the minimum two (2) year period of Movement of Youth (MOY) participation. Evidence must be submitted via the Confirmation of Community Service Form included in the application package. Forms should be completed for each project/service date and signed by a representative of the service provider. Applicants may make and submit as many copies as necessary.
Applicants must submit three (3) recommendations from a teacher, guidance counselor, and/or community service representative. Recommendation cannot be submitted from relatives of the applicant.
Incomplete application packets and/or materials received that do not adhere to the submission requirements, including submission deadline, will not be accepted and applicants will be disqualified. Please refer to the Application Checklist for required submission.
