Healthcare Districts are public entities that provide community-based healthcare services to residents throughout the state. They respond to the needs in their District by providing a range of services, which may include a hospital, clinic, skilled nursing facility, emergency medical services, and education and wellness programs.
Interfaith & FRHD Mobile Community Assistance Program
The Fallbrook Regional Health District has partnered with Interfaith Community Services' Mobile Community Assistance Program. This program provides mobile case management services to North San Diego County families with transportation issues. Two caseworkers will be in the van providing services. A total of 8 appointments are available: two clients on each hour.
The mobile unit is scheduled to be at our Center every second Tuesday of the month.
