Credentialing Requirements

Current resumes, along with copies of appropriate licenses and certifications relative to the provider's profession, must be submitted at the time of application and must be kept current throughout the contracting arrangement. Professional letters of recommendation and state-mandated clearances are also required.

Documentation regarding professional background, experience, credentials, licenses, and clearances may either be emailed directly to hr@ushealthcareservices.org, or faxed confidentially to the HR Department at 610.365.2397, and will be reviewed and responded to upon receipt. Questions may be directed to the HR Department, M-F, 8:30 a.m. - 4:30 p.m., at 610.365.2437. Documentation must include, but may not be limited to:

• Current Resume
• Copy of Professional License(s)
• Copy of Professional Certification(s)
• Act 168 Forms from all agencies/employers through whom you have worked with minors/children (see this link for more information)
• Current Act 24 Statement
• Current PA clearances, including FBI federal fingerprint clearance run through the Department of Education, PA Child Abuse, PA Criminal History, and PA Mandated Reporter Training Certificate (see links below)
• Current, negative TB test result or proof of treatment with current X-rays and a physician's certifying statement
• Completed application with minimum of two professional references (documents below)
