Parents of middle school students: if you would like to receive weekly reports on your child's Internet use, please use this form to request access to your child's relay information. Please list the district e-mail address for the student that matches the parent/guardian name in Power School. Please make sure you know your student's Andover student e-mail address (this will serve as a security checkpoint).

PLEASE NOTE: Information provided in this form will be cross-checked against the information we have in PowerSchool. If you are not the legal parent/guardian, or if the email information does not match the parent information, we will deny access. This is for security purposes. 

*You only need to do this once; your access will follow your student as they move grade levels.
