College Visit Verification Form

College Visit Form 

Students need to complete a College Visit Verification form PRIOR to attending a college visit.  Forms must be completed 3 days before the scheduled visits and students will need a parent's email to fill out the form. 

Attendance Information:

Juniors: Eligible for 1 college visits that will be marked as a school related activity
Seniors: Eligible for 2 college visits that will be marked as school related activity
