For safer and more efficient workflow in the UPCN Laboratory,
we are translating key processes to the digital platform.
To facilitate your request, download and fill up the following forms:
Submit these forms at least three (3) working days prior to your scheduled laboratory session to facilitate preparation of all materials. You can submit them through GSheet or in-person through the Laboratory Staff.
Note: For urgent requests DURING laboratory sessions, you can print the UPCN Lab Supply & Request Form 3 or request from our laboratory staff.
