This is the document I have received when I successfully transferred campus and the day I am a full-fledged UPLB student!
This document is helpful for every UPLB Computer Science student because this serves as their guide in accomplishing and checking if all the courses required in the program is accomplished.

Form 5 is the official record from the university that you are officially registered on your enrolled classes for that semester.
The form 5 also indicate your basic personal information, current standing in your program, and the total tuition cost you would need to pay if you are not under the RA 10931 or The Universal Access to Quality Tertiary Education Act.
Whenever you have taken a similar course in another UP constituent unit, you may accomplish this form in order to officially substitute the course you have taken to a course in your prescribed curriculum.
Personally, I have applied for substitution of courses in various Computer Science subjects, free electives, GE electives, and Human Kinetics subjects
