A Unique and Vital Part of the 

Western North Carolina Social and Health Care Systems

An Initiative Evaluation

Community Health Workers (CHWs) are front line public health workers that often come from the communities they work in. From January 2022 through December 2023 Mountain Area Health Education Center (MAHEC) worked with 12 Dogwood Health Trust funded organizations with embedded CHWs to implement community-based chronic disease management programing across Western North Carolina (WNC). MAHEC partnered with the North Carolina Center for Health and Wellness (NCCHW) Culture of Results Initiative to complete an evaluation of the intiative from January 2022 through December 2023. Desired Results of This Project

To understand, from the perspective of Community Health Workers and their clients, what makes them a critical part of the Western North Carolina social and  health care landscapes.
