What is the initial J/kg dose to defibrillate in cardiac arrest with ventricular tachycardia/fibrillation?

2 J/kg! Note that this higher than the dose needed to perform a synchronized cardioversion. After the initial 2 J/kg dose, if the patient remains in VT/VF, continue to escalate the dose, e.g. to 4 J/kg. Continue increasing as repeated shocks are needed, to a maximum of 10 J/kg. Think: 2, 4, 6, 8, that's the dose to defibrillate!

What is the compression:breaths ratio for 2-rescuer pediatric CPR, without an advanced airway?

  • 30:2

  • 15:2

  • Continuous compressions, breath every 2-3 seconds

When you have 2-rescuer CPR, your compression:breaths ratio should be 15:2. During 1-rescuer CPR, the recommended ratio is 30:2. After securing an advanced airway, you should transition to continuous compressions, with breaths every 2-3 seconds. Note that the increased breath rate is a new updated recommendation from the 2020 guidelines.
