Description: Learn from NACCHO (National Association of County and City Health Officials) staff about their community health planning resources and how you can use these for identifying and developing your diabetes strategy.
Description: Join a panel discussion with Maryland organizations that have learned how to connect with diverse groups of people in their communities. You will hear about the people and groups they have been able to reach and the challenges and successes of the methods they've used. The 90-minute session will provide ample time to ask questions and discuss methods that might work for different LHICs.

Description: Talk with representatives from the Maryland Primary Care Program (MDPCP) and CareFirst's Patient-Centered Medical Home (PCMH) program about how and why to connect with primary care practices in your county! These primary care practices are also working on diabetes prevention and management and can be partners as you implement your diabetes strategies. They can also be partners as your LHICs tackle other high priority topics. Discuss, ask questions, and connect!
Description: Building on the successes LHICs are having with their diabetes strategies, MDH sees an opportunity to expand the conversation into maternal and child health (MCH). Join us on Monday, July 19th for a special conversation with Shelly Choo, Director of the Maternal and Child Health Bureau at MDH and Cliff Mitchell, Director for the Environmental Health Bureau at MDH. They will present information about the Statewide Integrated Health Improvement Strategy (SIHIS) and the maternal and child health priority within it. You'll also be able to talk about MCH priorities in your communities. This conversation may identify new MCH opportunities that LHICs could pursue!
Description: The MDH Center for Chronic Disease Prevention and Control invites LHICs to a presentation and Q&A session on accessing population health data using BRFSS.
The Maryland Indicator Based Information System (IBIS) allows public access to data from the Maryland Behavioral Risk Factor Surveillance System (BRFSS) and the Maryland Youth Risk Behavior Survey (YRBS). Data includes the frequency of chronic diseases, risk behaviors, demographics, and other health outcomes; many data points are available at the state and local levels.
The MDH team will walk us through the IBIS query system (https://ibis.health.maryland.gov/) and provide tips that jurisdictions and their LHICs can use to leverage these data for LHIC planning purposes. Additionally, the MDH team will walk through available data tables focused on chronic diseases, risk factors and deaths at the county level.
Description: Join this convening to learn about the LHIC technical assistance structure and offerings through Sept 2022! During the convening, Pam Williams from the Maryland Department of Health’s Prevention and Health Promotion Administration will share the expectations for the year. The Horowitz Center will go into detail about the scope of their TA and the request process.
The Horowitz Center will also introduce a data guide that LHIC members can use when looking for information for needs assessments, identifying specific populations with health needs, planning activities and programs, and monitoring changes in health throughout the community.
