How does registering with Disability Services work?
Here is a general timeline:
A person decides they want to request accommodations.
The person gathers any documentation of their disability that they have available to them. If they have none, Disability Services will work with them to resolve this.
The person fills out the appropriate form:
The Student Intake Form for students and student employees, or
The Employee Request for Accommodations for employees who are not students.
The person shares any documentation of their disability with Disability services through one of the following methods:
Through a browser, or an app on a computer, Android, or iOS, the person can upload their documentation to OneDrive.
Through an app on Android or iOS, the person can share their documentation through Genius Scan.
The person can fax their documentation to Disability Services.
The person is contacted by Disability Services to arrange a meeting.
At that meeting, the person discusses with a Disability Services representative what accommodations they need, what documentation may need to be acquired, and their next steps.
Once accommodations are in place, Disability Services will work with the individual to implement them and provide support if issues arise.
How does requesting an event accommodation with Disability Services work?
Here is a general timeline:
A person decides they want to request accommodations. (This is ideally two or more weeks in advance.)
The person fills out the Event Accommodations Request Form
The person is contacted by Disability Services to confirm their accommodations.

How does registering with the Accessible Workplace Office work?
Here is a general timeline:
A person decides they want to request accommodations.
The person gathers any documentation of their disability that they have available to them.
The person completes the Reasonable Accommodation Request Form and sends it to the Accessible Workplace Office, either via:
Emailing the form to accessibleworkplace@umass.edu, or
Faxing the form to (413) 545-0802.
The person emails accessibleworkplace@umass.edu to arrange a meeting.
During the meeting, the Accessible Workplace Office works with the person to identify what documentation needs they have and which accommodations to put in place.
Once the accommodations are in place, the Accessible Workplace Office will provide support for the person if any issues arise.
