Parents and guardians are required to bring in the following documents to complete registration:
Proof of Age (e.g Birth Certificate, Birth Registration Form, Canadian Citizenship/Permanent Resident Card, Passport & Custodial documents if applicable).
Proof of Address (e.g. utility/tax bill, purchase/lease agreement, landline/cable/internet bill. A driver's license or bank/credit card statements are not accepted)
Verification of date of arrival/immigration documents (if child was not born in Canada)
Immunization records - please submit directly to Wellington Dufferin Guelph Public Health

If you are new to the Upper Grand District School Board, please complete this online form. You will need to create an account for UGDSB which will then give you access to information about your student once they are registered such as report cards, timetables, and more.

Out for Area (OOA) and Out of District (OOD) registration procedures for English-language track schools are governed in accordance with Policy 521 Admissions and Registration
Process for OOA / OOD Attendance:
The board will maintain a list of schools that are either 'open' or 'closed' to new OOA/OOD registrations.
Schools are reviewed each year and the list is updated and posted prior to the December break.
Only schools designated as 'open' for the coming school year can accept new OOA/OOD registrations.
New OOA/OOD registrations can only be submitted during a limited period of time during the month of January.
New OOA/OOD registrations must be completed by the last Friday in January.
Students registering at a school that is OOA or OOD may remain at the OOA/OOD school until completion of grades at that school, rather than having to reapply each year.
Transportation is NOT provided for OOA or OOD students.
We encourage students to attend their designated home school but do understand that there may be situations that lead to a request for students to attend a different school.
