   1. separate concussion form signed 

   2. updated physical. 

Physicals are good for 2yrs if dated April 1st or after. Another option for a quick physical, if you cannot get in to your regular physician, is CVS pharmacy across from Walmart in Kenosha or certain Walgreens have Minute clinics (see individual websites for more info and to make an appt online).
