Scholar Vaccination and Physical Requirements
ALL incoming Freshman (9th grade) and Transfer Scholars must have a current, up to date immunization record on file before the start of school. A current health physical is highly recommended as well.
Juniors (11th grade) must have a current health physical on file. The Commonwealth of Pennsylvania requires that ALL 11th grade students have a physical exam. ANY PHYSICAL DONE AFTER JUNE 2020 IS ACCEPTABLE.
Seniors (12th grade) must have documentation of 2nd Meningitis vaccine (meningococcal conjugate vaccine - MCV4) before the first day of school. Many scholars may have received their 2nd dose during their 16 year old well visit. A scholar receiving his/her first does after the age of 16 is acceptable as well. If a health physical was not given to the school within the scholars' 11th grade year, it is also required before the first day of school. ANY PHYSICAL DONE AFTER JUNE 2019 IS ACCEPTABLE.
Scholars in ALL grades (9-12) need the following vaccines:
• 4 doses of Diphtheria, Tetanus & Acellular Pertussis (DTaP) **(1 dose on or after 4th birthday)
• 4 doses of polio (1 dose on or after 4th birthday)
• 2 doses of measles, mumps & rubella**
• 3 doses of hepatitis B
• 2 doses of varicella (chickenpox) or evidence of immunity
• 1 dose meningococcal conjugate vaccine (MCV4)
• 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) [if five years have elapsed
since last tetanus immunization]
• 2nd dose of meningococcal conjugate vaccine (MCV4) TO ENTER 12TH GRADE
