When residents are dictating their discharge summary, they must cc the specific physicians’ name that they want their patient to follow up with so that these patients do not get lost in the system. For example, they must cc the specific Endocrinologist’s name or specific Cardiologists’ name as well as the specific location (clinic ie Heart Function clinic, Device clinic, Arrhythmia clinic, anticoagulation clinic, or ECC) where they will see the patient. This helps the unit clerks to not have to go searching for which clinics these physicians will be seeing the patients at because many of these physicians work at multiple locations. The specific clinic should also be written on the discharge summary.
