Step 1: Create an online enrollment account (this is different than a Parent Portal account) and fill out the applicable registration form:

After completing the online registration form (Step 1), families selected an appointment time. During your scheduled 20-minute appointment, you will meet with TES staff to review and submit all required registration documents. Please remember that all required documents must be provided at the appointment to finalize your child’s registration.
Online Enrollment is only Step 1. Registration is NOT complete until documents are submitted and the school confirms enrollment.

Child's birth certificate (an original, not a photocopy; we will make a photocopy and return the original to you).

Copy of Parent(s)/Guardian(s) photo ID
Most recent report card/transcript from your student’s previous school (if applicable)
Copy of signed IEP (if applicable)
Copy of 504 accommodation plan (if applicable)
Mass. Parental Notice with one-time consent to allow School District to access MassHealth (Medicaid) Benefits. (Not applicable for all students, but we ask parents/guardians to sign this form for each student.
Your child's up-to-date immunization record
The following Massachusetts Department of Public Health Immunization Requirements are needed before your child can enter kindergarten:
DTaP - Five doses of DTaP.
Polio - Four doses of polio vaccine.
MMR - Two doses of MMR.
Hepatitis B - Three doses of Hepatitis B.
Varicella - Two doses (or physician-certified reliable history of chickenpox)
In addition to the above regulations, the Massachusetts Department of Public Health also requires a current physical examination, including a history of lead poisoning screening as well as vision and hearing screening.
