1. Who Can Make a Referral?
Students can refer themselves.
Parents/Guardians can refer their child.
Teachers or Staff may refer a student for additional support.
2. How to Make a Referral
For Students:
Fill out a Student Self-Referral Form (paper or online).
Drop the form in the Counselor Referral Box outside the counseling office or submit it digitally.
Students can also ask a teacher to help contact the counselor.
For Parents:
Complete a Parent Referral Form (available online or in the school office).
Call or email the school counselor directly to request support for your child.
Attend scheduled parent conferences to discuss concerns and request follow-up.
For Teachers/Staff:
Use the internal Staff Referral Form or send an email to the counselor.
Include specific behaviors or concerns observed.
3. What Happens After a Referral Is Made?
The counselor reviews the referral within 1–3 school days.
The counselor will:
Schedule a check-in with the student.
Reach out to the parent/guardian if needed.
Determine if the student needs individual counseling, a small group, or outside referrals.
4. Types of Counseling Services Offered
Short-term individual support
Small group counseling (e.g., social skills, grief, conflict resolution)
Classroom guidance lessons
Referrals to outside agencies or community resources, if needed
5. Confidentiality Notice
Conversations with the counselor are confidential unless:
The student is in danger.
Someone else is in danger.
The student gives permission to share.
