2021-2022 Registration Forms
Click the desired registration form, download it, fill it out, print and sign/initial all appropriate boxes. After June 11th, please call the Choices Office at (559) 651-0155 to find out where to turn your completed registration forms in at. We will reach out to you and notify you of enrollment status as soon as possible. Parents are required to attend a parent orientation for students to begin program!

Choices Students/Parents.. what a year this has been! I never thought we would be in a virtual setting for after school and it has been quite an adventure. I pray we can see each other in person in August. I have had a blast with all of you. To my students, please know all of you are tremendously special to me. To your parents, thank you for trusting me and my team with your precious kiddos. Enjoy your summer and get lots of rest! Stay safe and stay healthy. I will see you in August.

Miss Sanchez's Summer Send Off
To my smart, wonderful, funny, third graders thank you for giving it your all during this year, I know that it has been tough for all of you. I want to thank you all for being such wonderful students and friends to each other. I enjoyed having each and every one of you in my class from Kenia’s song about how much she loves tacos, Camellia’s Hiiiii, Jaylene’s cat show and tells , and Aniyah’s correcting Ms.Sanchez on words she could not spell!! I am so proud of each and every one of you and can’t wait to see you as big 4th graders next year ! Have a wonderful summer until next year !

My fellow 4th/5th grade students, you all should be very proud of yourselves for surviving this crazy school year. Thank you for sticking it out this year and not letting this year totally flop! I am hoping that next school year will be better and will allow us to be back in person. Have a wonderful summer. Our last assignment is to do a fun scavenger hunt to see how much you can remember what we did in choices. Good luck and have fun. A New year comes with a New beginning.

SUMMER IS HERE!! 6th graders, you have made it through this school year! I hope you all are ready to start middle school, you all have overcome so many different challenges this school year and you have adapted to them all. I know that you all will have so much success in your future. For those of you who attended zoom sessions with me, I am so grateful for the opportunity to plan lessons for you!! Have an amazing summer and STAY COOL! :)
