The Intervention and Referral Services Team follows the state model in which (A) an initial referral is sent to the team, (B) the referral is reviewed by the team, and (C) additional I&RS forms are provided as needed. The teacher must complete all forms, provide work samples and collected data showing the strategies they have used to address the areas of need.
CLASSROOM INTERVENTIONS must be attempted and documented prior to a referral to the team.
After all the data and forms are submitted, the team leader will schedule a meeting after if it is determined the referral is appropriate, and complete when submitted. The meeting purpose is to develop an Action Plan that will be implemented, reviewed, and/or modified after a period of about 6-8 weeks for effectiveness.
The Intervention and Referral Team is a school resource process to support students in school success.
The team can provide resources to parents and teachers. The team is not automatically a step toward classification of students. Interventions and strategies must be demonstrated with data to support any other services that may be sought.
*Academically, for grades 2 and 3, students should present deficiencies 2-3 grade levels below their peers. Areas of attendance and behavior mat be a factor in the referral process and may be addressed through evidence to support the referral.
Record and maintain list with students of concern

4. Contact guardian(s) and speak to them about concerns, keeping track of all contact efforts.
5. Complete the COUNSELOR REFERRAL FORM (link below).
***Some cases warrant different solutions rather than I&RS.
Teacher must contact parent/guardian and inform them that they are referring their child to I&RS.

Please let them know they would have to attend I&RS meetings, agree to follow through with any at-home interventions assigned, and their consent would be necessary to refer the student to the Child Study Team for evaluation if it is determined that an Individualized Education Plan may be necessary.

Complete the grade-appropriate I&RS packet for their student (links below)
Incomplete forms will not be considered, Meetings will only be scheduled once COMPLETE packets are submitted
Send email to guidance counselor, Mrs. Chery (jchery@trenton.k12.nj.us), that you have completed and submitted a referral form along with all necessary collected data (electronic or hard copies of data accepted - Hard copies can be placed in counselor's mailbox).
Attend all I&RS meetings for referred student.

6. Once intervention period is over (6-9 weeks), we can determine if we will close a case, continue interventions with follow-up meetings as needed, or provide recommendation for submitting to Child Study Team for evaluation.

When might a teacher request an I&RS review?
A teacher routinely differentiates instruction to address a child’s needs in the classroom. The teacher requests services of the I&RS Committee when a particular child continues to have difficulties despite these efforts.
What kind of needs are reviewed by the I&RS Team?
When a child experiences difficulties that affect his or her academic progress, or has exhibited behavior that interferes with learning, the teacher may request support from I&RS. Student difficulties may include problems responding to written or verbal information, organizing, focusing, and/or completing work without constant teacher intervention.
How does I&RS help a teacher and student?
I&RS supports the teachers and students by developing an intervention plan that may provide alternative strategies, programs, and/or assessments. The interventions are designed to support the student in achieving success within the regular education program.
How are parents informed?
The teacher discusses his or her concern with the child’s parents prior to requesting an I&RS meeting. Once the teacher has requested an I&RS meeting for the child by submitting a completed referral packet along with supporting documents, parents will be invited to meet with the team and the classroom teacher to collaborate and provide input in the development of an action plan. Following the I&RS committee meeting, the parents will receive a copy of the completed action plan.
What happens during an I&RS meeting with the teacher?
Following the submission of an I&RS request to the team coordinator, the teacher is invited to discuss their concerns with the I&RS Team at a meeting scheduled during the school day. At that meeting, the teacher will describe the student and the challenge(s). The teacher will then identify both successful and unsuccessful intervention strategies that have been used for the student. Alternative means of intervention and new approaches are suggested. A plan of action is developed specifying the goals, strategies to be used to move toward goals, and the individuals responsible for each intervention action. A timeline is established for implementing the plan and assessing its effectiveness.
How is follow-up provided?
The action plan is monitored by the classroom teacher and the I&RS Committee. At a time specified within the plan, the teacher meets with the I&RS Team once again to report on results. If the student’s needs are not being met by the initial plan, additional interventions may be suggested and modifications made to the plan.
Is this the same as referral for Special Education?
NO. I&RS recommends actions intended to help resolve the challenge identified to prevent referral to the Child Study Team. If the actions taken and resources used are not adequate and the problem still remains, the child’s needs may suggest referral to the Child Study Team. Parents will participate in the decision as to whether a Child Study Team evaluation will be done.
Who can serve on the School’s I&RS Committee/Team?
Permanent members of the committee include the school administration, School Counselor, School Nurse, Teachers, ESL teacher, Literacy or Math coach/Learning Loss teacher, and/or *Speech Therapist and *CST Members.
