When you sign in with a non Google (gmail) account, you may encounter a grayed out page. 

It is Google Form not recognizing your account and requesting you to sign in with your Google (gmail) account.

If you do not have a gmail account, simply create one. After creating the account, it will allow you to register.

Proceed to key in your registration

In the “Comments”, if you wish, you may key in your preferred email address and we will change it for you.
