In order to receive food or supplies of any kind from The Daisy Project, every family must be registered.

Wishlist Item Request

We do keep a variety of collars, harnesses, leashes and grooming supplies in stock.  Please check our pantry location first before submitting a request.  Once your request has been submitted, please allow 7 days for us to fulfill your request. If it takes longer than 7 days, we will give you updates along the way.  

In addition, your item must be picked up within the next 2 open pantry days.  For example:  If we contact you on a Tuesday to pick up your item, you have that day and the next 2 times the pantry is open to pick up your items.  If you do not pickup your items within this timeframe, we will pass these items on to the next family. 

This form is for anything besides food or items we do not have available in our pantry.
