Program eligibility guidelines: Must be age 60 or older, Reside in Berks County, Income not to exceed $2,000/month for individual or $4,000/month for married individuals, Have heart-related or breathing related issues, Provide physician’s note reflecting the accepted related diagnosis/condition. Peerstar- Peer Support Services: https://www.peerstarllc.com/ This is a free community-based service through medical assistance for adults with mental health needs. Certified Peer Specialist (CPS) can assist individuals in accessing mental health, co-occurring and IDD services, referrals for competent doctors/other supports, assistance with older adult concerns, provide crisis support, develop natural supports, promote self-advocacy, support individuals in reconnecting with family, address basic needs of housing, employment, wellness and recovery, and improving compliance with probation and parole, etc. CPS also serve as a supportive person who can relate to mental health and provide resources and strategies for success.
