All responsible parties must sign and register with DWD. Students should print this form upon completion and return to their Youth Apprenticeship Specialist.

Program Requirements

Level One:

  • Junior or senior year of high school

  • 450 hours of paid work-based learning minimum

  • 2 semesters of related classroom instruction

Level Two (standard YA program):

  • Junior and senior year of high school

  • 900 hours of paid work-based learning minimum

  • 4 semesters of related classroom instruction

Students must be 16 years of age, in good standing academically, on track to graduate and meet the requirements of the employer
