Health Screenings

TEXAS STATE LAW MANDATES THAT ALL PRE-K, KINDER, 1ST, 3RD, & 5TH GRADE STUDENTS HAVE A VISION, HEARING, HEIGHT, WEIGHT, ACANTHOSIS NIGRICANS (1, 3, 5), AND SPINAL SCREENING (5TH FEMALES) COMPLETED AT SCHOOL. New students to the district will also receive these screenings per grade requirements.

**If you have reasons of conscience or religious reasons for refusing screening for your student on spinal or acanthosis please let me know and I can provide you the proper documentation to complete an affidavit.**

After screening is completed on a student; a referral will be sent home in the case that a student may need to follow up with their Pediatrician, Dentist, or Eye Doctor for further evaluation. The referral form has a portion to be filled out at the follow up appointment to be returned here in the clinic. The follow up portion of the referral will be used in school records to ensure we are providing the best possible care for your students and their ongoing education.
