COL – College Trip (After third visit) A – Absence (Code used by teachers)
ED – Early Dismissal
EX1 – Parent Note
EX2 – Doctor Note
EXC – Excused Absence
HBA – Homebound Absent
SUS – Out of School Suspension
Doctor’s Note After An Absence For Illness:
When a student’s absence for personal illness exceeds five consecutive days or a total of ten days in a six-week grading period, the student may be required to present a statement from a physician or health clinic verifying the illness or other condition requiring the student’s absence from school. If the student has established a questionable pattern of attendance, a physician’s or clinic’s statement may be required after a single day’s absence. [See FEC (LOCAL)] Forged or altered doctor statements may be reported to the authorities.
Regular Doctor And Dental Appointments:
If a student attends school before or after a doctor's appointment that same day and turns in the doctor's note, then the absence will be marked MED and will count the student present for the missed periods. To excuse a student's attendance for the full day, a note from the doctor's office must be turned in.
Any Other Reason The Student Was Absent:
When a student that was absent due to feeling ill or any other reason, a parent or the legal guardian must send a handwritten letter or can use the form provided down below to excuse an absences.
**All parent and doctor notes must be turned in within five school days after the student's return date. Notes can be turned in to the attendance office located in A1002 or can be sent via email to an attendance clerk.**

MED – Medical (student arrived late with a doctor note or received an early dismissal and turned in a doctor note)
OCI– On Campus Intervention
REL – Religious Holiday
SAR – SART (Meeting with Mrs. Sims)
T – Tardy
HBP – Homebound Present
CV1/CV2 – College Visit (Only for Juniors and Seniors, 2 per year)
MIL– Pursuing Enlistment
DRV– Obtaining DL or Permit
BUS– Tardy due to FWISD Bus
