TDCAA Championships: Friday October 24, 2025 @ Sunnybrook Park *tentative*

OFSAA Championships: Saturday November 1, 2025 @ Heart Lake, Brampton

Mandatory Requirements/Forms for TDCAA:

1. Eligibility – Completed online via the OFSAA HUB (speak to your AD/PE head) *should be done before first invitational meet. **Your AD must list your entire team on one roster – select varsity/co-ed.

2. Online Entries – via trackdatabase (see above)

3. Entry fee – Cheque turned in at registration of TDCAA meet. (see Fee Form above)

4. Supervision Form: A hardcopy must be turned in at registration of TDCAA meet (These will be couriered to the OFSAA committee for registration purposes – only TEACHER-COACHES listed can supervise/pick up package) (Link to form)

5. Student Officials: Most schools are responsible for bringing student officials to act as marshals either along the course or at the finish line. Click here for duties.

Mandatory Forms for OFSAA:

Submission deadlines: October 28, 2025 @ 11:59PM
Submission format: Certain forms to be scanned and uploaded to the OFSAA HUB (Athletic Director has access)
All athletes advancing to OFSAA must be included on/complete the required forms

Forms to be uploaded to the HUB:

Due October 28, 2025 @ 11:59 PM

1. Eligibility – was completed for TDCAA, must be printed off, signed by coach, AD, principal, scanned and uploaded to the HUB
2. Student – Waiver of understanding (One form per student. All forms uploaded into one file. Parent signature required)
3. Coach – Confirmation and Release (One form per coach. All forms uploaded into one file)
4. Para XC Registration Form (Only if you have para runners)

Forms Not Required to be uploaded but should be distributed and collected.

1. OFSAA Rules of Behaviour (one per athlete, parent signature required
2. Student-Athlete/Parent Concussion Review Confirmation (one per athlete, parent signature required)
3a. Participation Informed Consent and Assumption of Risk (Under 18) (one per athlete, parent signature required) OR
3b. Adult (18+) Release of Liability (one per athlete)
4. Photo Release Form (one per athlete, parent signature required)

**Practice Form is no longer required BUT the 16 practice minimum must still be met to be OFSAA eligible
