Below is the Medication Administration Packet. This includes the parent permission form and the doctor's order form. Feel free to ask your child's doctor's office to fax over an order for medications to be administered at school. Please remember that all medications (even over-the counter) need to have a doctor's order and parent permission form on file to be administered at school.  Thank you!
