South West Hospital and Health Service has three seeds — three big ideas to guide the future of our Community Mental Health, Alcohol and other Drugs Service.
We would like to understand what these 3 seeds mean to the unique and diverse communities across the South West — and to people like you.
Your story will be used to learn about the unique experiences of people in communities across the South West on the topic of Mental Health, Alcohol and Other Drugs
Your responses will be kept anonymous.
Quotes from your story will be shared with The project team which will include South West Hospital and Health Service staff, partners, consumers and carers and the team from The Australian Centre for Social Innovation — like me — who are facilitating this project.
At the end of the project, we will keep a text version of each response and delete any original voice recordings.
The app will ask for your permission to learn from and use your responses.

As part of our commitment to learning what works with community, we will be reaching out to a range of staff, community members, service partners and service stakeholders across the South West and asking them to have a conversation about what these three seeds mean to them.
