Benefits
For new hires, benefits are effective on the 31st day of employment.
Open enrollment elections are effective on January 1st each year.
Effective dates for elections following qualifying events are dependent on the date of the qualifying event.
Upon hire into a full time role, you have thirty days to enroll in our benefits plans. Your initial benefit selections take effect after 30 days of employment. You cannot change your elections outside of this period until the next Open Enrollment period OR if you experienced a qualifying life event. If you have had a life event change, please go to www.workforcenow.adp.com and click Myself → Benefits → Enrollments → Report a Life Change. You will be required to provide documentation for your life change, and the changes you are permitted to make to your benefits must align with the life event.
Open Enrollment is typically conducted once per year in the October-November time frame with an effective date of January 1.
Health, Dental, and Vision Benefits continue until the last day of the month in which your contract ends.
Voluntary coverages (disability, life insurance, etc) will end on the same day your contract ends.
You may elect to continue healthcare coverage under COBRA regulations. You will receive a COBRA Notice from our administrator, Ameriflex, via mail within 14 days of your last day, which will include instructions for continuing your coverage if you choose to do so.
Merrill Lynch is the investment advisor for the Synergis 401(k) Plan and ADP is the Plan Administrator.
You must be at least 21 years of age and have completed 6 months of employment. You can participate in the plan starting the first day of the month following the completion of the eligibility requirements.
Prior to your eligibility date, you will receive information regarding the enrollment process along with instructions for how to opt out of the plan if you do not want to participate. Once eligibility is met, you will be automatically enrolled in the plan default fund at a 3% contribution rate. If you would like to opt out of the automatic enrollment, you must actively decline enrollment. You may select an alternative deferral amount or to elect not to defer under the plan at any time.
You may make a Rollover Contribution of benefits from an eligible retirement plan to this Plan at any time. They are not subject to the 6 month of service waiting period. To initiate a rollover into our plan, attain the most recent statement from your previous employer or IRA. Then, contact our Merrill Lynch advisor, Alvaro Galvis - alvaro_galvis@ml.com, for assistance with the rollover process.
To transfer your existing Synergis 401(k) funds, contact ADP’s Participant Services Center at 800-695-7526.

Generally, hourly consultants are only paid for hours worked. However, eligibility for paid sick time or general paid time off can vary based on states and local regulations. If you are eligible, the applicable policy will be assigned to you in ADP. You can view policy assignments by logging into www.workforcenow.adp.com and going to Myself > My Time Off. Our current state and local time off policies can be found here. If you believe you are eligible for a paid time benefit and do not see the plan assignment in ADP, please contact humanresources@synergishr.com.
Personal Information Updates
Log in to workforcenow.adp.com. Click Myself → Profile → Personal Information → View More. Make the necessary edits to your details, and click Save.
Once your name change is legally approved, obtain updated identification documents like a driver's license, passport, and social security card with your new name. Then, email humanresources@synergishr.com and provide legal documentation supporting the change.
Miscellaneous
Use the “reset your password” or “forgot username” options in ADP. If you are still having trouble logging in to ADP, send an email to humanresources@synergishr.com for assistance.
Send an email to humanresources@synergishr.com requesting an employment verification letter.
Mineral is the name of the vendor that Synergis uses to administer training programs that are required by statute and/or by Clients. It is a web-based system, and you can complete the training online. So that you are compliant with regulations and Client requirements, please complete the assigned training programs within 14 days of starting your assignment.

Synergis is not authorized to employ Consultants in other countries. In rare cases, depending upon Client guidelines and local country ordinances, it may be permissible for our US-based employees to work overseas for brief periods of time (generally during scheduled vacations). If you wish to make a request for an alternate work location for a brief and limited period of time, you must obtain the approval of the Client as well as Synergis and complete the necessary forms for authorization well in advance of your planned travel.
