All New/Transfer students need to submit the following to the health office prior to starting school:
A physical signed by a health care provider (universal health care record)
Most recent immunization record
Any pertinent medical information or plans
Emergency Contact Information
Preschool Students only: require a dental screening (please reach out to your school nurse for more information)
Student who have medical concerns may be required to have an individual health care plan. Please see Forms for the Health Office tab or reach out to your school nurse for more information.
Student should stay home when sick to prevent the spread of germs or disease, please visit Guidance for Illness tap at the top of the page.
