Students with allergies that require rescue medication need to submit a Food Allergy and Anaphylaxis Emergency Care Plan (FARE) to the school nurse. This plan provides essential information to help school staff manage the student's condition in case of an allergic reaction.

Students with asthma/restrictive airway disease (RAD) need to submit an Asthma Action Plan to the school nurse. This plan provides essential information to help school staff manage the student's condition in case of a respiratory distress.

District Nurses provide the following School Health Services:
1. Vision and hearing screening.
2. Height and weight
3. Blood pressure
4. Preschool dental screening
5. Scoliosis screening – starting at age 10, to be completed every other year
6. Pediculosis - Preschool through 5th grade (two times per school year)

Students with seizure disorders need to submit a Seizure Action Plan (SAP) to the school nurse. This plan provides essential information to help school staff manage the student's condition in case of a seizure and outlines the appropriate steps to take during and after a seizure.

New students to the district are required to submit a physical from their health care provider (from within the calendar year).
4th grade students are strongly encouraged to submit a physical form (from within the calendar year).
All students can submit a universal health care record to keep the health office updated on the students physical status
